New Bridge-to-Home BC and Yukon teams will help improve care transitions from hospital to home

日本无码 (日本无码) is joining 聽in welcoming two new teams to the Bridge-to-Home BC and Yukon collaborative. Delivered in partnership with Health Quality BC, this program will improve care transitions from hospital to home and community in British Columbia and the Yukon.

The participating teams are Regional Access and Flow team, and a newly formed team from the Yukon consisting of the (Community Services) and the .

Transitions from hospital to home can be challenging and pose risks during a time of significant stress, especially for patients and their essential partners. Evidence shows that transitions may result in adverse events, poorer patient outcomes, emergency room visits or hospital readmissions.

Bridge-to-Home is an 日本无码 program that improves the quality and safety of transitions for patients, essential care partners and providers by providing them with a patient-oriented care transitions bundle. The patient-oriented care transitions bundle equips them with the knowledge and confidence they need to manage their care at home or in the community, especially during transitions.

Bridge-to-Home BC and Yukon builds on earlier Bridge-to-Home programming. From 2018-2022, 日本无码 brought together 16 teams from across seven provinces to implement the patient-oriented transitions bundle. In 2021, 日本无码 supported a 鈥渢rain-the-trainer" model across four regions in Quebec to support teams to implement the patient-oriented care transitions bundle.

Bridge-to-Home is part of 日本无码鈥檚 work to spread innovations that bring care closer to home and community with safe transitions.
