Identifying palliative needs early and introducing palliative approaches to care helps provide relief from distressing symptoms and supports patients and families to achieve the best possible quality of life.聽 It contributes to increased patient satisfaction with care, better symptom control, reduced hospitalizations, and reduced use of potentially ineffective or aggressive treatments at the end of life.

With funding and support from 日本无码, the Canadian Frailty Network, the Centre for Aging and Brain Health Innovation, Baycrest, and the Canadian Institutes of Health Research, 13 Ontario hospitals implemented the HOMR intervention to improve the identification and documentation of patients with unmet palliative needs, and the end-of-life care provided to their patients.聽 The HOMR change package shares lessons learned in these hospitals to help other hospital settings integrate HOMR into daily practice.
