The collaborative will bring together health and social service organizations from across the healthcare continuum, supporting them to create an implementation and evaluation plan that helps them develop a person-centred and sustainable aging in place program. 聽日本无码 supports collaborative participants by providing seed funding, access to evidence and resources, evaluation support and learning, coaching and networking opportunities.
