Partnering on Appropriate Virtual Care – Participating Teams

ÈÕ±¾ÎÞÂë is supporting 41 teams across 135 sites to participate in Partnering on Appropriate Virtual Care, an initiative that helps them work in partnership with patients, families, caregivers and communities to determine when and how virtual care should be used in their unique settings in an appropriate, safe and equitable way.

As part of their participation, each team is being supported to develop a functional framework for evaluating when and how virtual care can be used appropriately. The participating single or multi-site teams are located in nine provinces, representing urban, rural and remote settings. They serve diverse groups including vulnerable populations, older adults, First Nations, Inuit and Métis communities and newcomers to Canada.

Settings include:

  • primary care
  • community care
  • chronic disease prevention and management
  • mental health and additions
  • rehabilitation care such as physiotherapy and occupational therapy
  • long-term care
  • specialty care such as pediatric care, geriatric care and palliative care

Members of participating teams include:

  • a wide range of primary care providers including physicians, nurses, social workers, psychologists, physiotherapists and others
  • patient, caregiver and community members
  • leadership or executive sponsors
  • evaluation and project leads
  • representatives from all staff positions
